Falaremos do Servi\u00e7o de Gastroenterologia do HUCFF\/UFRJ, refer\u00eancia no estado e no pa\u00eds para o diagn\u00f3stico de doen\u00e7as complexas do trato gastrointestinal.<\/p>\n<p>Agradecemos a receptividade do Chefe do Servi\u00e7o, Prof. Dr. Homero Foga\u00e7a, e do Sub-Chefe, Dr. Cesar Amorim, que nos mostraram cada detalhe do Servi\u00e7o de Gastro do HUCFF, al\u00e9m da contribui\u00e7\u00e3o hist\u00f3rica da Prof. Associada Eponina Lemme, do Departamento de Cl\u00ednica M\u00e9dica da UFRJ e membro de Diretoria da AGRJ, que relembrou a trajet\u00f3ria percorrida pela Gastro\/HUCFF at\u00e9 hoje.<\/p>\n<p><a href=\"https:\/\/socgastro.org.br\/novo\/wp-content\/uploads\/2002\/07\/Construcao-4.jpg\"><img loading=\"lazy\" sizes=\"auto, (max-width: 1457px) 100vw, 1457px\" \/><\/a><\/p>\n<p>Inaugurado em mar\u00e7o de 1978, pelo ent\u00e3o reitor Lu\u00eds Renato Caldas, o Hospital Universit\u00e1rio Clementino Fraga Filho, localizado na Cidade Universit\u00e1ria, no Rio de Janeiro, tinha capacidade inicial de quase 500 leitos, 22 salas de cirurgias e ampla rede de ambulat\u00f3rios, al\u00e9m de CTI.<\/p>\n<p><a href=\"https:\/\/socgastro.org.br\/novo\/wp-content\/uploads\/2002\/07\/20211208_140334.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-15475\" iniciadas em 1950, por\u00e9m, foram paralisadas mais de uma vez por falta de recursos, at\u00e9 serem retomadas, em definitivo, em 1975. O que permitiu que, em 1978, o hospital fosse inaugurado.<\/p>\n<p>O pr\u00e9dio do HUCFF \u00e9 considerado um marco da arquitetura moderna e guarda em sua mem\u00f3ria a import\u00e2ncia da universidade p\u00fablica e do movimento estudantil na luta pela sa\u00fade e educa\u00e7\u00e3o.<\/p>\n<p>Atualmente, o Hospital Universit\u00e1rio Clementino Fraga Filho \u00e9 refer\u00eancia no tratamento de diversas doen\u00e7as, possuindo um campo tecnol\u00f3gico avan\u00e7ado e profissionais especializados em todas as \u00e1reas da sa\u00fade. Hoje s\u00e3o 150 leitos gerais e 150 leitos para tratamento de covid-19.<\/p>\n<p>Considerado um dos mais importantes\u00a0hospitais universit\u00e1rios\u00a0do\u00a0Brasil, recebeu este nome em homenagem ao\u00a0m\u00e9dico\u00a0e professor\u00a0Clementino Fraga Filho. O hospital, de alta e m\u00e9dia complexidade, presta assist\u00eancia aos pacientes do SUS (Sistema \u00danico de Sa\u00fade).<\/p>\n<p>Hoje, o Servi\u00e7o de Gastroenterologia e Endoscopia Digestiva \u00e9 refer\u00eancia no diagn\u00f3stico e tratamento de doen\u00e7as complexas do trato gastrointestinal.<\/p>\n<p>No inicio do servi\u00e7o de Gastro, a Hepatologia tamb\u00e9m fazia parte, ou seja, os dois servi\u00e7os funcionavam juntos, quando da inaugura\u00e7\u00e3o do HUCFF. Os profissionais envolvidos na chefia deste servi\u00e7o original de Gastroenterologia\/Hepatologia foram, respectivamente, os Professores: Jorge Toledo, M\u00e1rcio Cunha, Jos\u00e9 Fl\u00e1vio Ernesto e Henrique Sergio M. Coelho.<\/p>\n<p>Mas os prim\u00f3rdios da Gastroenterologia da UFRJ datam de antes da inaugura\u00e7\u00e3o do HUCFF, como relembra a Professora Associada Eponina Lemme, do Departamento de Cl\u00ednica M\u00e9dica da UFRJ:<\/p>\n<p>\u201cAntes de existir o Hospital Universit\u00e1rio Clementino Fraga Filho, o ensino da Cl\u00ednica M\u00e9dica era feito em v\u00e1rios hospitais da UFRJ. Havia a Cadeira de Cl\u00ednica M\u00e9dica do Prof. Clementino Fraga Filho, na Santa Casa, que contava com ambulat\u00f3rios e duas enfermarias. Outras Cadeiras pertenciam aos Professores Magalh\u00e3es Gomes e Carlos Cruz Lima, tamb\u00e9m na Santa Casa. Parte do ensino tamb\u00e9m era realizado no Hospital Moncorvo Filho, cujo titular era o Professor Luiz Feij\u00f3. Al\u00e9m disso, existia a 4\u00aa Cadeira, chefiada pelo Prof. Jos\u00e9 de Paula Lopes Pontes, no Hospital Escola S\u00e3o Francisco de Assis. Quando o HUCFF inaugurou, as atividades dos tr\u00eas hospitais migraram para o novo hospital escola, que recebeu o nome do Prof. Clementino Fraga Filho.&#8221;<\/p>\n<p>Dra. Eponina relata que, na d\u00e9cada de 80, o servi\u00e7o de Gastro era composto por duas enfermarias, ambulat\u00f3rios e, separadamente, um Servi\u00e7o de M\u00e9todos Especiais (SME), onde se realizavam os m\u00e9todos complementares, tais como: Endoscopia digestiva alta, Retossigmoidoscopia, Laparoscopia e os prim\u00f3rdios da Colonoscopia.<\/p>\n<p>\u201cInicialmente, o SME foi dirigido pelo Prof. Sergio Viegas de Andrade, rec\u00e9m chegado dos Estados Unidos, onde ficara quatro anos. Era um gastroenterologista completo, afinal, al\u00e9m de cl\u00ednico, realizava todos os m\u00e9todos at\u00e9 ent\u00e3o dispon\u00edveis. Tamb\u00e9m introduziu no Servi\u00e7o, o equipamento de Motilidade Esof\u00e1gica (Manometria), sendo eu a sua 1\u00aa disc\u00edpula. Infelizmente, o Prof. Sergio Andrade veio a falecer dois anos depois. Ent\u00e3o, pelo que me recordo, a chefia do SME, passou pelas m\u00e3os de outros professores, dentre eles: Profs. Glaciomar Machado, Marcus Tulio Haddad, Jos\u00e9 Flavio Coelho e Cl\u00e9ber Vargas\u201d, conta Profa. Eponina.<\/p>\n<p>Quando os servi\u00e7os de Gastro e Hepatologia se separaram, a chefe j\u00e1 era a Professora Celeste Elia, que foi respons\u00e1vel pelo grande crescimento do servi\u00e7o. Quando se aposentou, o Professor Homero Foga\u00e7a assumiu o cargo e o faz at\u00e9 hoje.<\/p>\n<p>\u201cEm 2005, a Prof. Celeste Elia, titular de Gastroenterologia, identificou uma \u00e1rea ociosa no 4\u00ba andar do hospital. Levantou recursos, com iniciativa privada, e construiu o servi\u00e7o que temos hoje, com a incorpora\u00e7\u00e3o do SME-Gastro. Adquiriu v\u00e1rios aparelhos e construiu tr\u00eas \u00e1reas separadas de exames, a saber: para a endoscopia convencional, para o laborat\u00f3rio de es\u00f4fago e para os m\u00e9todos mais avan\u00e7ados, tais como: CPRE, ecoendoscopia, etc. Sempre investindo em moderna tecnologia. A prof. Celeste Elia criou o Doutorado na Gastroenterologia, consolidou as linhas de pesquisa, investiu nas ag\u00eancias de fomento para aquisi\u00e7\u00e3o de equipamentos mais modernos e implementou cursos de especializa\u00e7\u00e3o, cujos recursos foram utilizados na manuten\u00e7\u00e3o das condi\u00e7\u00f5es prediais do Servi\u00e7o e na manuten\u00e7\u00e3o e aquisi\u00e7\u00e3o dos equipamentos. Um audit\u00f3rio ligado ao servi\u00e7o tamb\u00e9m foi constru\u00eddo. Ela exerceu a chefia de forma brilhante at\u00e9 se aposentar. Os chefes e os m\u00e9dicos que constru\u00edram a hist\u00f3ria da Gastro da UFRJ sempre foram muito zelosos, demonstrando grande orgulho por pertencerem ao servi\u00e7o\u201d, ressalta Dra. Eponina Lemme, que hoje \u00e9 professora colaboradora aposentada.<\/p>\n<p>Atualmente sob a chefia do Prof. Dr. Homero Foga\u00e7a, o Servi\u00e7o est\u00e1 em um patamar ainda mais elevado, tendo se tornado, o melhor ou um dos melhores do Brasil, considerando-se Hospitais Universit\u00e1rios, na opini\u00e3o da Dra. O perfil do atendimento costuma ser de pacientes mais complexos, com doen\u00e7as cr\u00f4nicas e neoplasias malignas do tubo digestivo.<\/p>\n<p>\u201cVale ressaltar que os procedimentos endosc\u00f3picos t\u00eam se tornado as principais ferramentas e s\u00e3o indispens\u00e1veis para o diagn\u00f3stico e tratamento das doen\u00e7as do aparelho digestivo mais prevalentes, como o c\u00e2ncer colorretal e o c\u00e2ncer g\u00e1strico, que se encontram entre as 5 neoplasias mais frequentes. A hemorragia digestiva, complica\u00e7\u00e3o oriunda de doen\u00e7as agudas ou cr\u00f4nicas, tamb\u00e9m demanda m\u00faltiplas a\u00e7\u00f5es endosc\u00f3picas, v\u00e1rias em car\u00e1ter emergencial\u201d, explica Dr. Homero.<\/p>\n<p>Tanto os pacientes internados, como aqueles atendidos em ambulat\u00f3rio, s\u00e3o submetidos a exames endosc\u00f3picos de m\u00e9dia e alta complexidade, diagn\u00f3sticos e terap\u00eauticos, eletivos ou emergenciais. O servi\u00e7o oferece vagas \u00e0queles em acompanhamento regular no HUCFF e tamb\u00e9m a pacientes de outras Unidades Hospitalares do Munic\u00edpio do Rio de Janeiro ou de outros munic\u00edpios do estado, via SISREG e NIR-HUCFF.<\/p>\n<p>O Sub-Chefe do Servi\u00e7o, Dr. Cesar Amorim, explica que no 1\u00ba e 2\u00ba andar funcionam os ambulat\u00f3rios:<\/p>\n<p>\u201cTemos ambulat\u00f3rios de todas as subespecialidades da Gastro. Desde a gastroenterologia geral \u00e0 ambulat\u00f3rios especializados em enfermidades do es\u00f4fago, p\u00e2ncreas e intestino. Atuamos fortemente nas famosas doen\u00e7as inflamat\u00f3rias intestinais, inclusive, cinco ambulat\u00f3rios s\u00e3o dedicados exclusivamente \u00e0 este tema\u201d, comenta.<\/p>\n<p>Os exames mais realizados no servi\u00e7o s\u00e3o Endoscopia e Colonoscopia. Por dia, somam em torno de 20 endoscopias e de 10 a 12 colonoscopias. H\u00e1 cerca de um ano o servi\u00e7o come\u00e7ou a realizar a enteroscopia por c\u00e1psula endosc\u00f3pica, algo totalmente in\u00e9dito no Rio de Janeiro.<\/p>\n<p>\u201cRecentemente, adquirimos um enterosc\u00f3pio de duplo bal\u00e3o, que \u00e9 outra ferramenta usada para investiga\u00e7\u00e3o do intestino delgado, e viramos refer\u00eancia regional junto com\u00a0 a aquisi\u00e7\u00e3o da c\u00e1psula endosc\u00f3pica. S\u00f3 a gente possui este equipamento no estado do Rio, e no Brasil. Apenas outros dois locais possuem esta c\u00e1psula. Tamb\u00e9m realizamos, por semana, 20 CPRE\u00b4S\u201d, comenta Dr. Cesar.<\/p>\n<p>A excel\u00eancia do servi\u00e7o tamb\u00e9m \u00e9 resultado da pol\u00edtica de renova\u00e7\u00e3o de equipamentos por editais de pesquisa.<\/p>\n<p>\u201cEm 2008, por exemplo, conseguimos atrav\u00e9s de um apoio de projeto de pesquisa da FAPERJ, comprar um ecoendosc\u00f3pio, o que era extremamente novo na \u00e9poca. As doen\u00e7as do intestino delgado sempre foram de dif\u00edcil avalia\u00e7\u00e3o e com as novas tecnologias passaram a ser mais identificadas. Ou seja, ao longo da nossa caminhada, fruto do trabalho em equipe e dos investimentos, aumentamos o nosso n\u00edvel de abrang\u00eancia\u201d, explica Dr. Cesar.<\/p>\n<p>&#8220;Temos curso de p\u00f3s-gradua\u00e7\u00e3o, mestrado, doutorado e de aperfei\u00e7oamento com atua\u00e7\u00e3o em Gastroenterologia Cl\u00ednica e Endoscopia Digestiva, o que tamb\u00e9m garante a nossa qualidade&#8221;, frisa Dr. Homero.<\/p>\n<p>Em 2020, apesar das mudan\u00e7as ocasionadas com a pandemia de covid-19, o Servi\u00e7o de Gastroenterologia do HUCFF se manteve produtivo. Devido ao aumento da demanda assistencial, o servi\u00e7o abriu duas novas salas de atendimento ambulatorial geral e, em 2021, houve a necessidade de abertura de mais uma para o acompanhamento de pacientes com doen\u00e7a inflamat\u00f3ria intestinal.<\/p>\n<p>\u201cDurante a pandemia, o hospital conseguiu arrecadar recursos e reformou todo o 1\u00ba e 2\u00ba andar. As paredes pintadas e os novos assentos foram um esfor\u00e7o da dire\u00e7\u00e3o em arrecadar fundos tanto na esfera p\u00fablica quanto privada. Houve um ganho enorme nas condi\u00e7\u00f5es estruturais dos ambulat\u00f3rios e das enfermarias\u201d, afirma Dr. Cesar.<\/p>\n<p>Hoje, o servi\u00e7o conta com duas enfermarias com cinco leitos cada, sendo uma masculina e outra feminina, 18 salas com turnos de ambulat\u00f3rios gerais e especializados, Laborat\u00f3rio de Es\u00f4fago, composto por tr\u00eas salas de exames, Setor de M\u00e9todos Especiais (SME-Gastro), composto por seis salas de exames, e atendimento permanente de urg\u00eancia aos pacientes internados nas emerg\u00eancias hemorr\u00e1gicas, \u00e0 noite, e tamb\u00e9m aos finais de semana e feriados.<\/p>\n<p>No Laborat\u00f3rio de Es\u00f4fago s\u00e3o realizados pHmetria, manometria tradicional e de alta resolu\u00e7\u00e3o, impedanciopHmetria, videofluoroscopia da degluti\u00e7\u00e3o, videoesofagografia e ultrassonografia intraluminal esofagiana.<\/p>\n<p>J\u00e1 no Setor de M\u00e9todos Especiais, s\u00e3o realizados exames de endoscopia alta, colonoscopia, colangiopancreatografia endosc\u00f3pica (CPER), ecoendoscopia e diversos procedimentos endosc\u00f3picos (dilata\u00e7\u00f5es endosc\u00f3picas, gastrostomias, polipectomias, mucosectomia, etc.), bi\u00f3psias hep\u00e1ticas e medidas de gradiente de press\u00e3o do sistema porta, al\u00e9m da enteroscopia por videoc\u00e1psula endosc\u00f3pica.<\/p>\n<p>O sucesso atual do servi\u00e7o se deve tamb\u00e9m, em grande parte, \u00e0s lideran\u00e7as passadas, como a da Dra. Celeste Elia, professora titular aposentada.<\/p>\n<p>&#8220;Ela assumiu a chefia do servi\u00e7o em 2005 e reestruturou o todo o modelo de atendimento, com uma administra\u00e7\u00e3o focada no paciente e mais empreendedora. A partir dela, o n\u00famero de atendimentos da gastroenterologia aumentou vertiginosamente&#8221;, frisa Dr. Homero.<\/p>\n<p>\u201cEu ainda era aluno da faculdade da Gastro, quando em 2005, houve uma nova etapa no servi\u00e7o. Dra. Celeste foi um marco na nossa hist\u00f3ria, uma revolu\u00e7\u00e3o! Nordestina de pulso forte, ela rompeu padr\u00f5es e fez negocia\u00e7\u00f5es com a dire\u00e7\u00e3o do hospital. Ela conseguiu um espa\u00e7o no 4\u00ba andar, que estava vazio, derrubou tudo e com apoio e verba de doa\u00e7\u00f5es construiu uma nova etapa no servi\u00e7o. Dando origem \u00e0 sala de m\u00e9todos especiais, t\u00e3o importante atualmente. Inclusive, o nome do nosso anfiteatro, que tem capacidade para 40 pessoas, foi em sua homenagem&#8221;, recorda Dr. Cesar.<\/p>\n<p>Outra melhoria foi a instala\u00e7\u00e3o de fibras \u00f3ticas, que ligam as salas de atendimento aos pacientes ao audit\u00f3rio reformado. Assim, os alunos conseguem acompanhar os exames endosc\u00f3picos e os procedimentos em tempo real. E como ressalta Prof. Dr. Homero: \u201c\u00e9 uma forma de evitar a exposi\u00e7\u00e3o do paciente sem prejudicar a qualidade do ensino.\u201d<\/p>\n<p>O servi\u00e7o ainda conta com um bisturi de arg\u00f4nio, que em alguns casos, evita a realiza\u00e7\u00e3o de procedimentos e cirurgias maiores em detrimento de procedimentos mais simples.<\/p>\n<p>A novidade do servi\u00e7o \u00e9 que em breve atender\u00e1, atrav\u00e9s do SISREG, \u00e0 solicita\u00e7\u00f5es de Enteroscopia por C\u00e1psula Endosc\u00f3pica.<\/p>\n<p>Orgulhosa por ter vivenciado essa hist\u00f3ria de sucesso, Dra Eponina Lemme, finaliza com um balan\u00e7o dos anos trabalhados com a equipe da Gastro:<\/p>\n<p>\u201cMuitos professores se aposentaram por idade e v\u00e1rios m\u00e9dicos foram contratados, todos de padr\u00e3o excelente, o que nos orgulha muito.
